Sample Analysis Request Form (SARF) – Samples submitted to the laboratory must be accompanied by appropriate labels, documentation, and instructions for testing. The Sample Analysis Request Form (SARF) is used to document each sample submission to our laboratory and should accompany the sample shipment.

Please submit samples directly to the laboratory that will be performing the testing. Samples are received Monday – Friday, except holidays, during normal daytime business hours. If you have any questions, please contact the laboratory location for details.
